Analysis of Malignant Ovarian Tumors with Second Look Operation




Abstract
A clinical trial was done to evaluate the efficacy of anticancer chemotherapy in malignant ovarian tumor and the clinical utility of tumor associated marker. 30 cases during a 41/4 years from January 1986 to March 1990 were confirmed as malignant
ovarian tumors pathologically, which were performed anticancer chemotherapy till adequate clinical response, then second look operation was performed.
The result revealed that negative second look operation rate was noted 94.44% at stage I. 66.67% at stage II and 50% at stage III and the total negative second look operation rate was 80%.
According to size of residual tumors after initial operation, negative second look operation rate was noted 100% in cases of less than 2 cm in diameter and 33.33% in cases of exceeding 2 cm.
Tumor associated marker CA 125 was noted 30.77% at stage I, 33.33% at stage II and 71.43% at stage III, but the CA19-9 showed no clinical correlation with staging. The combined positive rate of tumor associated marker CA 125 and CA 19-9 revealed
that
46.15% at stage I, 66.67% at stage II and 71.43% at stage III, which showed much higher positive rate than single tumor associated marker and by histologic type of epithelial ovarian tumor, 66.67% for serous cystadenocarcinoma, 50% for mucinous
cystadenocarcinoma and 50% for endometriod adenocarcinoma.
The positive rate of tumor associated marker as tracing examination for positive second look operation showed 50%.
We suggest that more advanced ovarian cancer and more residual tumor volume responded less effectively to anticancer chemotherapy and that tumor associated marker. CA 125 and CA19-9 combination revealed as sensitive modality in predicting
epithelial
ovarian cancer and in tracing examination, but in tracing examination the positive rate of tumor associated marker for positive second look operation was 50%, so other tracing modalities as like as second look operation would be needed.
